If the problem with Toyota is consistent, why aren't all cars of the model having the same problem?

If the problem is with firmware only, then the version number(s) should be known, right?

If the problem is a combination of software and firmware, then the version(s) number(s) of both should be known, right?

Is it a driving pattern?

Is it environmental conditions?

Is it environmental conditions combined with driving patterns?
